How to Spell Words during Communication. (Phonetic Alphabets)

In English some words seem to be similar to others while hearing on phone or without seeing the lip movement of the other person while he is speaking. Normal flow will be easy to understand but words out of normal flow seem to be confusing. For example ‘b’ and  ‘d’, ‘b’ and  ‘v’, ‘m’ and ‘n’, ‘b’ and ‘p’.
